We plan to accomplish this by providing assessments, community support, individualized plans of care, individual interventions, specialized Client Specific Training and referrals to other agencies.

Autumn Lake LLC will strive effortlessly to improve the quality of life for our elderly and disabled population by teaching skills to assist clients to function effectively at home and in the community. We will help families to understand the service plan that our experienced team will develop alongside the client and their family to reduce and prevent hospitalization.
